In terms of Section 38 of the Labour Relations Act, public service
bargaining councils can refer disputes to the CCMA.
What is The Purpose of This Form?
To refer a dispute between 2 or more bargaining councils in the
public sector to the CCMA.
Who Fills in This Form?
Any party to the dispute.

A copy of the form must be served on the other party.
Proof that a copy of the form has been served on the other party
must be supplied by attaching -
- a copy of a registered slip from the Post Office; or
- a copy of a signed receipt if hand delivered; or
- a signed statement confirming service by the person delivering the form; or
- a copy of a fax confirmation slip; or
- any other satisfactory proof of service.
