What is The Purpose of This Form?
This form requests the Minister of Labour to extend the period of,
or renew, a collective agreement that has been extended to
non-parties.
Who Fills in This Form?
The Secretary of the Bargaining Council.
Instructions
Two completed copies of this form must be sent to the Department
of Labour.
Three signed copies of the Certificate of Representativeness of
parties to the agreement or of the Council, must accompany this
form.
