The Basic Conditions of Employment Act requires emloyers to give
their workers a Certificate of Servive when their employment is
ended.
What is The Purpose of This Form?
To serve as proof that the worker worked for the employer.
Who Fills in This Form?
The employer.
Instructions
Employers must give this form to workers when they end their
employment.
The reason for ending the employment must only be completed if
workers ask for it.
