The Basic Conditions of Employment Act requires employers to keep
record of the attendance of their workers.
What is The Purpose of This Form?
To keep a record of attendance.
Who Fills in This Form?
The worker, or if the worker is unable, the employer.
Instructions
Employers must keep this form for three years after the last
entry.
If employers keep this record, they do not have to keep any other
records about their workers' working time or pay.
