Form EEA11 - Application for an Employment Equity Report

Anyone can request an employment equity report of a certain employer from the Department.

What is The Purpose of This Form?

To request an employer's employment equity report from the Department of Labour

Who Fills in This Form?

The applicant.


When they send the form, applicants should also include a -
  • revenue stamp to the value of R15; and
  • self-addressed (postage paid) envelope.
